• Patients undergoing long-term narcotic or other medication management
• Headaches - noncervicogenic in origin (e.g. migraines, tension)
• Pelvic pain
• Fibromyalgia
• Patients with significant untreated or poorly treated psychological problems, such as depression, anxiety disorder, malingering, personality disorder.
• Patients requiring detoxification or patients with or suspected of drug abuse.
